Important Information for Membership Chairs

National PTA Official Back to School Kit: Remind your President to register online here for this wonderful resource tool if they have not already done so.

The Unit President will need the unit's six digit National Unit Code to order. (This can be found on the front page of your unit bylaws or contact your Region Membership Chair.)

Form A & Insurance: A unit must have filed the new Form A and paid their insurance before the membership packet will be mailed to the unit President.

Membership Card Confirmation: Make sure this (yellow reply form) is completed online at http://www.nyspta.org/forms/membershipcardconfirm.cfm as soon as you have received your cards.

Membership Log: Keep an accurate list of all individuals who paid dues to your PTA. A sample membership log is provided on the NYS web site here.

Membership Cards: Each card should carry the National PTA unit ID, the member's name, the unit name & 5 digit NYS PTA unit number (19-xxx), and the expiration date (all cards expire on September 30 of the following year).

Membership Card Template: A membership card template is available on the NYS web site here. Save it to your computer or merge it with your member list, or type it directly on the template to print the membership cards. If any cards are destroyed during printing, they must be returned to your region membership chair immediately.

Additional Cards: If required, may be obtained from your region membership chair. Refer to Section 6 (Membership), page 8 of the NYS PTA Resource Guide for instructions and conditions on how to.

Dues Payments:

1st dues payment must be received at the state PTA office on or before November 1.

Mail dues to the state office monthly: with a dues payment form. Forms can be downloaded and printed here. Your unit code number must be on all correspondence. Remember two (2) signatures are required on all PTA checks.

Final membership payment must be received at the state PTA office no later than March 31st.

DO NOT DESTROY OR DISCARD MEMBERSHIP CARDS! Each card is worth $4. Unused/voided or damaged cards must be returned to your region membership chair by March 31st.
